    1. Find your property’s legal description found on the most recent deed.
      • The deed can be found by searching the property owner’s name at the following link: https://franklin.oh.publicsearch.us/.

    2. Print and complete all pages in the Change of Name forms PDF.

    3. In front of a Notary Public, sign the Change of Name forms and have it notarized.

    4. You will need to write a check for recording fees made payable to Franklin County Recorder’s office. To calculate the recording fees, please visit: https://recorder.franklincountyohio.gov/Resources/Fees.
      • Please note – Only the Affidavit of Name Change Form will be recorded along with your property legal description. Do not include the Change of Name Form in your recording fee calculation.

    5. Mail all forms, a self-addressed and stamped envelope, and the check for recording fees. Mail all items to:
